Step 1: Read Frequently Asked Questions for applicants.

This document contains important information that you must be aware of at the time of application. Please pay particular attention to the timeframes required for processing applications and allow sufficient time for completion of the credentialing process before planning to begin your practice.

Step 2: Read Bylaws, HCA Code of Conduct, Mission, Vision and Values Statement.

Application Attachments, Bylaws, HCA Code of Conduct, Mission, Vision and Values Statements: These are unique to Gulf Coast Medical Center. All applicants must complete the application attachments and review the other items. Applicants who wish to dual-apply (at both hospitals) must contact BMC at (850) 747-6937 for additional documentation and instructions.

Step 3: Pay application fee.

The application fee can be paid by check, or online via PayPal if you wish to pay by credit or debit card. Applications that do not include the application fee will not be processed until the fee has been received. (see FAQ, step 1) for information about reduced fee amounts for telemedicine radiology and temporary privileges.

**Reduced application fee is $250

Step 4: Return application and other materials for processing.

Review the application checklist to be sure all necessary steps have been taken and all items have been included. Return the completed application and other required items to:

  • To expedite processing, you may fax or scan and email the application and attachments so that initial processing can begin before the original is received.
  • By accepting and completing an application and returning it with the application fee, you acknowledge and understand that you have reviewed the Medical Staff Bylaws and Code of Conduct. You further agree to be bound by same, both at the time of application and throughout your appointment and reappointment terms. YOU ARE STRONGLY URGED TO PRINT THE BYLAWS FOR FUTURE REFERENCE AND BE FAMILIAR WITH THEM.

Step 5: Await confirmation that application has been received.

The application will be thoroughly reviewed and processed. You should receive an acknowledgement by email within one week of receipt. The acknowledgement will advise you any missing information and may request clarification or additional materials. It is the burden of the applicant to provide any necessary information requested in order to complete the credentialing process. Applicants can expedite the process by informing the affiliations and references listed in the application to promptly reply to the hospital when questionnaires are sent to them. An interview with the Credentials Committee may be requested before a recommendation is made. Applicants will be informed within one week of the Board of Trustee's decision on the application.
